Balancing Honesty and Hope in Fertility Care

Dr. Bareh shares her philosophy on balancing realistic expectations with genuine hope — and how compassionate, honest communication guides every step of fertility care at LLFAW.

In reproductive endocrinology and infertility (REI), every patient conversation carries both medical and emotional significance. At Loma Linda Fertility & Wellness Center (LLFAW), our philosophy is centered on combining evidence-based fertility care with compassionate, individualized communication.

Fertility treatment is not only about protocols and outcomes—it is about guiding patients through uncertainty with clarity, honesty, and support.

Transparent Expectations in Fertility Treatment

From the very first consultation, I prioritize setting realistic expectations. Each treatment plan—whether it involves ovulation induction, intrauterine insemination (IUI), in vitro fertilization (IVF), or advanced reproductive technologies—is discussed with clear explanation of expected success rates and contributing factors.

This transparency is not meant to discourage patients. Rather, it is designed to ensure that every decision is made with full understanding, allowing patients to move forward with confidence and trust in the process.

Maintaining Hope While Staying Grounded

Fertility care is rarely linear. Many patients experience setbacks before achieving success, and it is important not to interpret early challenges as final outcomes.

At LLFAW, I focus on helping patients stay engaged in their care while we continuously reassess and optimize treatment strategies. If there is a reasonable path forward, I encourage persistence, thoughtful adjustments, and individualized protocol refinement.

At the same time, I am committed to honesty. It is just as important to recognize when a treatment approach is no longer effective as it is to continue when there is meaningful potential for success.

The Role of Emotional Support in REI Care

One of the most difficult moments in fertility medicine is delivering a negative pregnancy test. These results are emotionally painful for patients—and never routine for the physician providing care.

In these moments, my approach is to remain steady, compassionate, and focused. While empathy is essential, patients also benefit from calm guidance that helps them process the outcome and understand the next steps clearly.

After acknowledging the emotional impact, we focus on what the result means clinically and what options remain available moving forward.

Guiding Next Steps After a Negative Cycle

When treatment is unsuccessful, I encourage patients to take time to process the result before making major decisions. Fertility care decisions are best made with clarity rather than in moments of emotional distress.

We typically regroup shortly afterward to review the cycle, identify opportunities for improvement, and discuss next steps. This structured approach helps patients move forward with a sense of direction rather than uncertainty.

Our Philosophy at LLFAW

At Loma Linda Fertility & Wellness Center, our goal is to ensure that every patient feels informed through transparent, evidence-based counseling, supported through every stage of treatment, empowered to make decisions aligned with their goals, and guided with both honesty and hope.

Fertility care requires both scientific precision and human understanding. Our commitment is to provide both—consistently and compassionately.
